Abstract
Objective To investigate factors associated with postoperative complications after thoracolumbar fracture surgery using binary logistic regression analysis.Methods In this retrospective study,206 patients with thoracolumbar fractures who underwent percutaneous kyphoplasty at The Third Provincial People's Hospital of Henan from June 2018 to June 2023 were enrolled.According to the occurrence of postoperative complications,patients were divided into a complication group(n=23)and a non-complication group(n=183).The incidences of postoperative pneumonia,poor incision healing,and cardio-cerebrovascular complications were recorded.Clinical data were compared between the two groups.Binary logistic regression was used to identify factors associated with postoperative complications after thoracolumbar fractures.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of each factor for postoperative complications.Results Among the 206 patients,23(11.17%)developed postoperative complications,including 10 cases of pneumonia,10 cases of poor incision healing,and 3 cases of cardio-cerebrovascular complications.Compared with the non-complication group,the complication group had older age and longer operative time,and higher proportions of unstable fractures,antibiotic use without indication,and comorbid diabetes mellitus;all differences were statistically significant(P<0.05).Binary logistic regression analysis showed that advanced age,longer operative time,unstable fracture,and comorbid diabetes mellitus were factors associated with postoperative complications after thoracolumbar fractures(P<0.05).ROC curve analysis showed that the areas under the curve(AUC)for age,operative time,unstable fracture,and comorbid diabetes mellitus in predicting postoperative complications were 0.792,0.803,0.636,and 0.611,respectively.The AUCs for age,operative time,and unstable fracture were>0.500 with statistically significant differences(P<0.05).Although the AUC for comorbid diabetes mellitus was>0.500,the difference was not statistically significant(P>0.05).Conclusion There is a certain risk of postoperative complications after thoracolumbar fractures.The main associated factors include advanced age,longer operative time,unstable fracture,and comorbid diabetes mellitus.These factors are associated with the occurrence of postoperative complications and may be used for perioperative risk assessment and targeted monitoring.关键词
