Abstract
Objective To analyze the predictive value of N-terminal pro-brain natriuretic peptide(NT-proBNP),creatine kinase-MB isoenzyme(CK-MB),and serum amyloid A(SAA)in children with severe pneumonia complicated by heart failure(HF).Methods A total of 214 children with severe pneumonia admitted to Kaifeng Children's Hospital between March 2022 and March 2025 were enrolled.Eighty children whose severe pneumonia progressed and was complicated by HF were assigned to the HF group,and 134 children without HF were assigned to the non-HF group.Baseline characteristics,hematological indicators,and other parameters were compared between the two groups.Results Univariate analysis showed that the white blood cell count(WBC),lactate dehydrogenase(LDH),NT-proBNP,CK-MB,and SAA levels in the HF group were significantly higher than those in the non-HF group.Multivariate analysis showed that NT-proBNP,CK-MB,and SAA were independent risk factors for HF in children with severe pneumonia.Predictive value analysis showed that the areas under the receiver operating characteristic curve(AUCs)of NT-proBNP,CK-MB,SAA,and their combined detection for predicting HF were 0.756,0.748,0.730,and 0.866,respectively.Conclusion Combined detection of NT-proBNP,CK-MB,and SAA has certain predictive value for predicting HF in children with severe pneumonia.关键词
