Abstract
Objective To investigate the clinical efficacy,cosmetic improvement,and safety of fluocinolone acetonide and hydroquinone tretinoin cream combined with intense pulsed light(IPL)in the treatment of moderate to severe melasma.Methods A total of 96 patients with moderate to severe melasma who underwent cosmetic treatments in Nanyang Central Hospital from January to August 2025 were conveniently selected.They were randomly divided into a laser group(n=48)and a combined group(n=48)using a random number table.The laser group received only intense pulsed light(IPL)therapy,while the combined group additionally applied fluocinolone acetonide,hydroquinone,and tretinoin cream topically before bedtime.The treatment period lasted 12 weeks,followed by a 3-month follow-up.The melasma area and severity index(MASI)scores,objective cosmetic parameters(melanin content,skin glossiness,skin elasticity),and treatment efficacy were compared between the two groups before treatment,at 12 weeks,and at 3-month follow-up.Adverse reactions and recurrence rates were also recorded.Results All 96 patients completed all treatment courses and follow-up.There was no statistically significant difference in MASI scores and various skin beauty indicators between the two groups before treatment(P＞0.05);After a 3-month follow-up after treatment,the total effective rate of the combined group was 93.75％,significantly higher than the laser group's 85.42％,and the difference between the groups was statistically significant(Z=1.976,P=0.048);The MASI score and melanin index of the combined group were significantly lower than those of the laser group(P＜0.001);The skin glossiness and elasticity were significantly higher in the combined group than in the laser group(P＜0.001);The incidence of adverse reactions in the combined group was 4.17％,which was lower than that in the laser group 16.67％,(x2=4.019,P=0.045);The recurrence rate of the combined group was 2.08％,which was lower than that of the laser group at 18.75％.The difference between the groups was statistically significant(x2=7.144,P=0.008).Conclusions The combination of Fluocinolone acetonide hydroquinone and tretinoin cream with strong pulsed laser therapy has significant therapeutic effects,high safety,and low recurrence rate in the treatment of moderate to severe melasma,and is worthy of clinical promotion and application.关键词
