陈斌教授从肝脾论治自身免疫性肝病相关性贫血的经验
Professor Chen Bin's experience in treating autoimmune liver disease-associated anemia from the liver-spleen perspective
摘要
Abstract
The spleen and stomach are regarded as the foundation of postnatal life and the source of qi and blood production,as the generation of qi and blood is closely associated with the spleen's transforming and transporting functions.Anemia may arise during the course of autoimmune liver disease(AILD),which is one of its extrahepatic manifestations.Based on twenty years of clinical experience in hematological conditions, professor Chen Bin proposes that the core pathogenesis of this disease involves"liver depression and spleen deficiency, mutual binding of damp-heat and stasis-toxin,and deficiency of qi and blood".Although the disease location is in the liver, its root lies in the spleen. In clinical practice,he emphasizes"simultaneous treatment of the liver and spleen",guided by the principle of"supp orting healthy qi and eliminating pathogenic factors while addressing both the root and the branch".The therapeutic approach prioritizes"reinforcing the earth to strengthen the root"that is,fortifying the spleen and supplementing qi to enhance the source of qi and blood generation-while also incorporating methods to soothe the liver and relieve stagnation, clear heat and drain dampness, and dissolve stasis and remove toxins, thereby tackling the secondary manifestations.Significant clinical efficacy has been observed, fully demonstrating the advantage of holistic pattern differentiation and coordinated regulation of the liver and spleen in traditional Chinese medicine. This article summarizes Professor Chen's diagnosis and treatment experience, accompanied by a validated case example for reference.关键词
