Association of serum GDF-15 and HIF-1α levels with glucose and lipid metabolism in patients with gestational diabetes mellitus and their predictive value for adverse pregnancy outcomes

Abstract
Objective:Gestational diabetes mellitus(GDM)refers to a hyperglycemic state that first occurs during pregnancy in women with normal glucose metabolism prior to conception.In recent years,its incidence has shown an increasing trend and may have multiple short-and long-term adverse effects on maternal and neonatal health.This study aims to explore the association of serum growth differentiation factor-15(GDF-15)and hypoxia-inducible factor-1α(HIF-1α)levels with glucose and lipid metabolism in GDM patients,and to analyze their predictive value for adverse pregnancy outcomes. Methods:A total of 82 GDM patients admitted to Huanghe Sanmenxia Hospital from January 2019 to July 2023 were retrospectively selected as a GDM group,and 83 pregnant women with normal antenatal examinations during the same period were selected as a healthy group.All patients in GDM group received standardized exercise and medical interventions,and were further divided into a good pregnancy outcome subgroup(n=45)and an adverse pregnancy outcome subgroup(n=37)according to pregnancy outcomes.Serum GDF-15 and HIF-1α levels were measured by enzyme-linked immunosorbent assay(ELISA).Glucose metabolism indicators,including fasting plasma glucose(FPG),fasting insulin(FINS),glycated hemoglobin(HbA1c),and lipid metabolism indicators,including total cholesterol(TC)and triglyceride(TG),were tested using a fully automatic analyzer.Pearson correlation was used to assess the association of GDF-15 and HIF-1α levels with glucose and lipid metabolism.Receiver operator characteristic(ROC)curve analysis was performed to evaluate the predictive value of GDF-15 and HIF-1α for adverse pregnancy outcomes. Results:Serum GDF-15 and HIF-1α levels in the GDM group were significantly higher than those in the healthy group(all P<0.05),and the FPG,FINS,HbA1c,TC,and TG levels in the GDM group were also significantly higher than those in the healthy group(all P<0.001).Serum GDF-15 and HIF-1α levels in GDM patients were positively correlated with HbA1c,TC,and TG levels(all P<0.001).Serum GDF-15 and HIF-1α levels in the adverse pregnancy outcome subgroup were significantly higher than those in the good outcome subgroup(all P<0.001).The combined predictive value of GDF-15 and HIF-1α levels was superior to the predictive value of either indicator alone(both P<0.05). Conclusion:Serum GDF-15 and HIF-1α levels are significantly elevated in GDM patients and are closely associated with glucose and lipid metabolism disorders.The combination of GDF-15 and HIF-1α levels shows good predictive value for adverse pregnancy outcomes.关键词
