Abstract

Objective:Post-stroke patients frequently suffer from residual balance and gait dysfunction,which severely affects quality of life and rehabilitation prognosis.Bobath therapy promotes postural control through sensory input,but its efficacy remains controversial;the cord-strapping technique corrects abnormal posture via external mechanical support.The combined use may produce synergistic effects,but the specific impact on balance and gait function in post-stroke hemiplegia patients requires further validation.This study investigated the effects of the combined application of cord-strapping technique and Bobath therapy on balance and gait function in stroke-recovery patients. Methods:A total of post-stroke hemiplegia patients treated at the Rehabilitation Medicine Center of First Affiliated Hospital of Nanjing Medical University from January 2023 to September 2024 were enrolled.Patients were randomized using a random number table a the conventional group(n=20),a Bobath group(n=20),and a combination group(n=20).All 3 groups received standard rehabilitation.The Bobath group received additional Bobath-based manual sensory input therapy,and the combination group received cord-strapping technique on top of Bobath therapy.Before and after treatment,lower-limb motor and gait function were assessed using the Fugl-Meyer Assessment-Lower Extremities(FMA-LE)and the 10-Meter Walk Test(10MWT).Balance function was evaluated using the Timed Up and Go Test(TUGT),Berg Balance Scale(BBS),and posturographic parameters including envelope area and total sway-trajectory length measured by a balance-function assessment and training system. Results:Before treatment,there were no statistically significant differences among the conventional,Bobath,and combination groups in FMA-LE(13.10±2.94,13.75±2.88,and 13.30±2.39),10MWT(38.10±5.92,38.09±6.33,and 38.58±5.46),TUGT(22.38±3.05,22.78±3.24,and 22.60±3.78),BBS(10.25±2.05,10.50±2.19,and 10.40±2.06),envelope area(10.40±1.58,10.59±1.81,and 10.64±1.85),and total sway-trajectory length(10.42±1.73,10.49±1.92,and 10.19±1.62).After 4 weeks of treatment,FMA-LE(17.95±2.68,20.80±3.43,and 24.95±3.14),10MWT(31.05±5.77,27.42±5.07,and 23.53±5.61),TUGT(19.05±3.50,17.96±2.18,and 15.33±2.12),BBS(20.15±2.70,24.50±2.19,and 28.45±2.16),envelope area(7.91±1.62,6.71±1.12,and 5.67±1.39),and total sway-trajectory length(8.85±1.80,7.85±1.43,and 6.55±1.42)were all significantly improved compared with baseline(all P<0.001).Between-group comparisons showed that both the Bobath group and the combination group outperformed the conventional group in improving FMA-LE,10MWT,TUGT,BBS,envelope area,and total sway-trajectory length(all P<0.001),and the combination group showed superior efficacy compared with the Bobath group for all outcomes(all P<0.001). Conclusion:The cord-strapping technique combined with Bobath therapy significantly improves balance and gait function in post-stroke hemiplegia patients.This combined therapy enhances intrinsic postural-control ability and optimizes motor patterns through external assistance.It has the advantages of simple operation,reliable efficacy,and ease of clinical adoption,providing a new comprehensive intervention strategy for rehabilitation in stroke-recovery patients.
