Differentiation and Treatment of Diabetic Peripheral Neuropathy Based on the"Latent Toxin"Theory

Abstract
Based on the"latent toxin"theory,the core pathogenesis of diabetic peripheral neuropathy(DPN)involves deficiency of healthy qi,endogenous production of toxic pathogens,their prolonged latency within the collateral vessels,and consequent damage to the sinews and channels.It can be categorized into three phases:the latent toxin concealment phase,the toxic pathogen outbreak phase,and the toxin-induced deficiency and damage phase.In the latent toxin concealment phase,the pathological mechanism is characterized by deficiency of qi and yin as the root cause,with initial accumulation of phlegm and stasis as the secondary manifestations.Toxins begin to form but are not yet intense.Treatment should focus on reinforcing healthy qi and dispelling pathogenic factors,with detoxification and unblocking the collaterals as the core principles,aiming to eliminate toxins in their incipient state,prevent further damage to the collat-erals,so Modified Qi-Replenishing and Toxin-Resolving and Collateral-Dredging Decoction can be used.The toxic pathogen outbreak phase is a critical period marked by intense toxin activity,obstruction of the collateral vessels,and a severe struggle between pathogenic and healthy qi.The pathological features involve intermingled toxin and stasis,leading to congestion and stagnation in the collaterals.Treatment follows the basic principle of expelling toxin outward,resolving stasis,and unblocking the collaterals.Medication adheres to the principle of detoxifying without harming healthy qi,resolving stasis without consuming blood,and unblocking collaterals without dam-aging yin.Through the dual actions of expelling toxin and removing stasis,the damage of toxin to the collaterals is alleviated,and the balance of qi,blood,yin,and yang is restored,so Modified Toxin-Expelling and Stasis-Resolving and Collateral-Dredging Decoction can be adopted.The pathological characteristics of the toxin-induced deficiency and damage phase can be summarized as severe deficiency of healthy qi,incomplete clearance of residual toxins,and damage to the collaterals with sinew atrophy.Treatment is based on the princi-ple of reinforcing healthy qi and expelling toxin,integrating three methods:supplementation,resolving turbidity,and tissue repair,so Modified Decoction for Strengthening Healthy Qi and Dispelling Toxins and Restoring Collaterals can be adopted.关键词
