Analysis of tongue manifestation patterns and syndrome correlation in Li Youwei's differentiation of peeling coating
摘要
Abstract
Objective:To investigate the characteristics of peeling coating and its correlation with traditional Chinese medicine syndromes in Li Youwei's diagnosis.Methods:Statistical analysis was conducted on 100 patients with peeling coating regarding TCM syndromes,tongue quality(tongue shape,tongue color),coating quality,coating color,and peeling locations.Results:The main TCM syndromes were yang deficiency with dampness obstruction,yang deficiency with phlegm coagulation,yin deficiency with fire hyperactivity,yin deficiency with internal dryness,and yin deficiency with phlegm-fire.Red tongue was predominant in tongue quality,with tender and thin tongue shapes being most common.White and yellow coating were predominant.positive correlations were found between:white tongue and yang deficiency with dampness ob-struction/phlegm coagulation;red tongue and yin deficiency with internal dryness/phlegm-fire;crimson tongue and yin defi-ciency with fire hyperactivity;enlarged/tooth-marked tongue and yang deficiency patterns;thin tongue and yin deficiency with fire hyperactivity;tender/aged tongue and yin deficiency with internal dryness;white thick coating and yang deficiency patterns;white greasy coating and yang deficiency with phlegm coagulation;white slippery coating and yang deficiency with dampness obstruction;thin yellow/no coating and yin deficiency with fire hyperactivity;scorched yellow coating and yin defi-ciency with internal dryness;yellow greasy coating and yin deficiency with phlegm-fire(r>0,P<0.05).Negative correlations(r<0,P<0.05)were observed between red tongue and yang deficiency with dampness obstruction/yang deficiency with phlegm coagulation;pale white tongue and yin deficiency with fire effulgence/yin deficiency with internal dryness/yin defi-ciency with phlegm-fire;crimson tongue and yin deficiency with phlegm-fire;thin tongue/tender tongue and yang deficiency with dampness obstruction/yang deficiency with phlegm coagulation;enlarged tongue/teeth-marked tongue and yin defi-ciency with fire effulgence/yin deficiency with internal dryness/yin deficiency with phlegm-fire;scorched yellow coating and yang deficiency with dampness obstruction/yang deficiency with phlegm coagulation/yin deficiency with phlegm-fire;white thick coating/yellow greasy coating and yin deficiency with fire effulgence/yin deficiency with internal dryness.Peeling coat-ing in yang deficiency patterns,yin deficiency with phlegm-fire,and yin deficiency with phlegm-heat primarily manifested in the middle tongue region,with qi stagnation potentially affecting the edges.Yin deficiency with fire effulgence exhibited denudation in the middle,edge,and root regions,often presenting as a coatingless tongue,while yin deficiency with internal dryness mainly showed anterior denudation.Conclusion:Peeling coating is commonly seen in yin deficiency patterns but also occurs in yang deficiency patterns,demonstrating characteristic tongue manifestations across different syndrome pat-terns.Significant correlations exist between tongue features and syndrome differentiation:Peeling coating in yin deficiency typically presents as a red,thin,dry tongue,while in yang deficiency it manifests as a pale,enlarged,moist tongue.This cor-respondence indicates that tongue examination parameters can serve as important references for TCM syndrome differentia-tion,providing guidance for precise diagnosis and treatment planning.关键词
