"国考"导向下手术并发症经济负担的循证研究
Evidence-based study on the economic burden of surgical complications under the guidance of national public hospital performance evaluation:a PSM empirical model based on a tertiary hospital in Tianjin

Abstract
Objective Under the context of the National Performance Evaluation of Tertiary Public Hospitals,this study aimed to quantify the economic burden of postoperative complications on both patients and hospitals,providing evidence for opti-mizing the prevention and control strategies of surgical complications.Methods Based on data from a tertiary hospital in Tian-jin,16,355 eligible surgical patients admitted between January 2023 and June 2025 were included.A 1∶4 propensity score matching(PSM)nearest neighbor matching method was used to control for confounding factors,and differences in hospitalization costs were compared between the complication group(n=790)and the non-complication group(n=3,160).Results The in-cidence of postoperative complications was 4.83％.The per capita total cost in the complication group was 48,100 yuan,repre-senting a 20.9％increase compared to the non-complication group(P＜0.001),with out-of-pocket expenses 37.8％higher.The total cost in the non-normal complication group was 1.25 times that of the non-complication group,with an average daily indi-rect economic loss of 5,815.18 yuan.Conclusion Surgical complications significantly exacerbate the economic burden on pa-tients,particularly complications related to medical errors.There is an urgent need to establish a closed-loop management system of"prevention-monitoring-intervention"through perioperative refined management,informatized monitoring and early warning,and multidisciplinary collaboration mechanisms,aiming to achieve dual improvements in medical quality and economic efficiency.关键词
