超声乳化+人工晶体植入联合房角分离术治疗闭角型青光眼合并白内障的临床疗效
The clinical efficacy of phacoemulsification with intraocular lens implantation plus goniosynechialysis in the treatment of angle-closure glaucoma complicated with cataract

Abstract
Objective To explore the clinical efficacy of phacoemulsification with intraocular lens im-plantation plus goniosynechialysis in the treatment of angle-closure glaucoma(ACG)complicated with cataract.Methods A retrospective analysis was conducted on the clinical data of 120 patients with ACG combined with cataract admitted to our hospital from May 2021 to April 2025.They were divided into Group A(trabecu-lectomy,n=61)and Group B(phacoemulsification with intraocular lens implantation plus goniosynechialysis,n=59)according to the treatment methods.The clinical efficacy of the two groups was compared.The corneal endothelial functions including endothelial cell density(ECD),average cell area(ACA),and corneal central thickness(CCT).And surgery-related indicators before and 3 months after the operation,as well as the ocular structural and functional indicators before and 1 month and 3 months after the operation were compared.The incidence of complications(anterior chamber hemorrhage,anterior chamber inflammation,corneal edema,transient intraocular pressure increase)and the recurrence rate 12 months after surgery were recorded.Results The clinical efficacy of Group B was higher than that of Group A(P<0.05).Three months after the operation,ECD in Group B was higher than that in Group A,and ACA and CCT were lower than those in Group A(P<0.05).The operation time and postoperative hospital stay in Group B were both shorter than those in Group A(P<0.05).The visual field defect,best corrected visual acuity,central anterior chamber depth and retinal nerve fiber layer thickness of Group B were all better than those of Group A at 1 month and 3 months after operation(P<0.05).There was no statistically significant difference in intraocular pressure levels be-tween the two groups(P>0.05).The total incidence of complications in Group B was lower than that in Group A(P<0.05).At 12 months after the operation,there was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusion Phacoemulsification with intraocular lens implantation plus goniosynechialysis has a better therapeutic effect on ACG combined with cataract.It can shorten the opera-tion time and hospital stay,facilitate the recovery of ocular structure,improve corneal endothelial function,and has good safety.关键词
