睡眠情况与缺血性中风血瘀证的关联研究
Correlation Study Between Sleep Status and Ischemic Stroke with Blood Stasis Syndrome

Abstract
Objective:To explore the correlation between sleep disorders and acute ischemic stroke with blood stasis syndrome and provide a theoretical basis for the prediction of blood stasis syndrome in ischemic stroke.Methods:A total of 922 in-patients with acute ischemic stroke admitted to the department of encephalopathy,the First Affiliated Hospital of Guangxi University of Chinese Medicine from January 1,2019 to May 31,2024 were en-rolled.According to the Diagnostic Criteria for Syndrome Differentiation of Stroke(Trial),the patients were divided into the blood stasis syndrome group and the non-blood stasis syndrome group.The Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the sleep status of the patients,and the National Institutes of Health Stroke Scale(NI-HSS)was adopted to assess the degree of neurological deficit.Results:The score of blood stasis syndrome in pa-tients with acute ischemic stroke was positively correlated with sleep efficiency(r=0.123,P＜0.001),sleep distur-bance(r=0.070,P=0.034)and total PSQI score(r=0.085,P=0.01).After adjusting for age,gender,marital sta-tus,history of hypertension,history of diabetes,smoking history and drinking history,increased scores of sleep qual-ity[OR=1.31,95％CI(1.02,1.70),P=0.037],sleep efficiency[OR=1.34,95％CI(1.09,1.63),P=0.005]and to-tal PSQI score[OR=1.06,95％CI(1.00,1.12),P=0.04]might increase the risk of blood stasis syndrome in acute ischemic stroke.Meanwhile,elevated scores of sleep quality[β=1.03,95％CI(1.00,1.06),P=0.037],sleep effi-ciency[β=1.04,95％CI(1.01,1.06),P=0.004]and total PSQI score[β=1.01,95％CI(1.00,1.01),P=0.04]were all associated with higher scores of blood stasis syndrome in acute ischemic stroke.In addition,it was found that sleep latency in patients with blood stasis syndrome of acute ischemic stroke was positively correlated with NI-HSS score(r=0.239,P=0.011).After adjusting for confounding factors including age,gender,marital status,histo-ry of hypertension,history of diabetes,smoking history and drinking history,prolonged sleep latency could aggra-vate the degree of neurological deficit in blood stasis syndrome of ischemic stroke[β=1.87,95％CI(1.12,3.13),P=0.017].Conclusion:Poor sleep status is not only a risk factor for blood stasis syndrome in ischemic stroke,but also closely related to the functional recovery of patients with blood stasis syndrome in acute ischemic stroke.关键词
