便携可视喉镜在夜间院前急救中的应用效果和安全性研究
Application Effect and Safety of Portable Video Laryngoscope in Pre-hospital Emergency Care at Night
摘要
Abstract
Objective To investigate the efficacy and safety of a portable video laryngoscope for endotracheal intubation in critically ill patients during night-time prehospital emergency care.Methods Forty critically ill patients requiring prehospital emergency endotracheal intubation admitted to our hospital from January 2023 to January 2025 were selected as the study subjects.They were randomly divided into an observation group(n=20)and a control group(n=20)using a random number table.The control group underwent intubation using a traditional laryngoscope,while the observa-tion group underwent intubation using a portable video laryngoscope.The first-attempt success rate,overall success rate,glottic exposure time,total intubation time,changes in hemodynamic parameters[heart rate(HR),mean arterial pressure(MAP)]at different time points before and after intubation,and the incidence of complications were compared between the two groups.Results The first-attempt success rate(85.00%vs.50.00%)and the overall success rate(95.00%vs.70.00%)in the observation group were significantly higher than those in the control group(P<0.001).The glottic expo-sure time and total intubation time in the observation group were significantly shorter than those in the con-trol group(P<0.001).Repeated measures analysis of variance showed that the time effect and interaction effect for both HR and MAP were statistically significant in both groups(P<0.05).At immediately after intubation and 2 minutes after intubation,the levels of HR and MAP in the observation group were significantly lower than those in the control group(P<0.05).Furthermore,the total incidence of complications in the observation group was significantly low-er than that in the control group(10.00%vs 35.00%,P<0.05).Conclusion In night-time prehospital emergency care,compared with the traditional laryngoscope,the portable video laryngoscope can significantly improve the success rate and efficiency of endotracheal intubation,effectively reduce the hemodynamic impact on patients,and lower the risk of compli-cations,demonstrating higher clinical application value and safety.关键词
