Analysis of 42 cases of adverse drug reactions in children induced by anti-seizure medications and pharmaceutical care recommendations

Abstract
Objective:Epilepsy is a common chronic neurological disorder in childhood.Anti-seizure medications(ASMs)are the mainstay of treatment;however,long-term use is prone to adverse drug reactions(ADRs),which seriously affect treatment adherence and quality of life in pediatric patients.This study aims to analyze the utilization of ASMs in children and the characteristics of related ADRs,in order to provide data support and practical references for rational drug use and ADR prevention and control in clinical practice. Methods:A retrospective study was conducted using the hospital medical safety adverse event reporting system.Clinical data of pediatric patients who developed ADRs associated with ASMs at Hunan Children's Hospital from January 2018 to December 2021 were retrieved and collected.Information including sex,age distribution,route of administration,time to ADR onset,implicated drugs,specific clinical manifestations of ADRs,intervention measures,and outcomes was collected and analyzed using descriptive statistical methods. Results:A total of 42 pediatric patients with ASMs-related ADRs were included,aged from 6 months to 15 years,including 29 males(69.05%)and 13 females(30.95%).Among the implicated drugs,oxcarbazepine accounted for the highest proportion of ADRs(19 cases,45.24%),followed by phenobarbital(10 cases,23.81%)and lamotrigine(6 cases,14.29%).The remaining 7 cases(16.66%)involved other drugs,including levetiracetam and sodium valproate.The systems/organs most commonly affected by ADRs were skin,liver,and hematologic system.After the occurrence of ADRs,37 patients discontinued the suspected drugs,switched to alternative medications,and received symptomatic treatment.All patients improved and were discharged after intervention,and no serious adverse outcomes occurred. Conclusion:ADRs induced by ASMs are commonly associated with aromatic ASMs and present with diverse clinical manifestations,with skin,liver,and hematologic system injuries being the most frequent.In clinical practice,monitoring throughout the entire course of ASM therapy should be emphasized,particularly strengthening pharmaceutical care in the early stage of treatment,to promptly identify and intervene in ADRs and thereby ensure the safety and effectiveness of anti-seizure therapy in children.关键词
