低频重复经颅磁刺激对基底节区脑梗死后构音障碍的疗效研究
Therapeutic effect of low-frequency repetitive transcranial magnetic stimulation on dysarthria after basal ganglia infarction

Abstract
Objective To investigate the therapeutic effect of low-frequency repetitive transcranial magnetic stimulation(rTMS)on dysarthria after basal ganglia infarction.Method Sixty patients with non-thrombolytic cerebral infarction who were admitted within 24 h of their initial onset were randomly divided into rTMS group(n=30)and sham stimulation group(n=30),both receiving conventional treatment.After the condition stabilized,the rTMS group underwent low-frequency rTMS treatment,while the sham stimulation group received sham stimulation.The modified Frenchay dysarthria assessment scale(Frenchay scale)was used to assess dysarthria,and the maximum phonation time(MPT),voice loudness,and speech intelligibility were measured and recorded.Result The total effective rate in the rTMS group was significantly higher than that in the sham stimulation group(χ2=10.80,P=0.001).Compared with those in the sham stimulation group,there were no significant differences in Frenchay scale scores,MPT,voice loudness and speech intelligibility before treatment(all P>0.05),Frenchay scale scores,MPT,voice loudness and speech intelligibility were significantly increased after treatment and at 2 and 4 weeks after treatment(all P<0.01).Frenchay scale scores,MPT,voice loudness and speech intelligibility after treatment and 2 and 4 weeks after treatment in the rTMS group were significantly higher than those before treatment(all P<0.01),and Frenchay scale scores,MPT,voice loudness and speech intelligibility at 2 and 4 weeks after treatment were significantly higher than those after treatment(P<0.05-0.01).Conclusion rTMS can significantly improve language function in patients with dysarthria after basal ganglia infarction with stable therapeutic effect,and further improvement in long-term dysarthria.关键词
