经皮三叉神经半月节球囊压迫术与射频热凝术治疗老年人原发性三叉神经痛的疗效与安全性对比研究
Comparative study of the efficacy and safety of percutaneous balloon compression versus radiofrequency thermocoagulation for primary trigeminal neuralgia in elderly patients

Abstract
Objective To compare the efficacy,safety,and complications of percutaneous balloon compression(PBC)of the trigeminal semilunar ganglion with percutaneous radiofrequency thermocoagulation(RFT)of the trigeminal semilunar ganglion in elderly patients with primary trigeminal neuralgia(PTN),thereby providing evidence for clinical decision-making.Methods The clinical data of 78 elderly patients with PTN admitted to the Department of Pain,Lianyungang Second People's Hospital from January 2022 to October 2022 were analyzed retrospectively.According to the surgical method,40 cases were divided into PBC group and 38 cases were divided into RFT group.Preoperative and postoperative visual analogue scale(VAS)pain scores,overall efficacy,complications,and recurrence rates were recorded and analyzed.Results There was no significant difference in preoperative VAS scores between the two groups.At 1 hour postoperatively,VAS scores in both groups decreased significantly(P<0.05),with no significant difference at this time point.The overall effective rate was 95%in the PBC group and 97.4%in the RFT group,with no significant difference(P>0.05).The incidence of moderate to severe facial numbness was higher in the RFT group than in the PBC group(P<0.05).Differences in masticatory weakness,decreased corneal reflex,and oral herpes between groups were not significant(P>0.05).Recurrences occurred in both groups,with no significant difference in recurrence rates(P>0.05).Conclusions Both PBC and RFT have their respective advantages and disadvantages in treating elderly patients with PTN.Both procedures effectively relieve pain,with facial numbness being the most common complication.PBC offers better relief of facial numbness but requires general anesthesia and incurs higher costs.关键词
