Study on the Medication Rule of Tianma(Gastrodia Rhizoma)Formula Based on"Disease-Drug-Quantity"

Abstract
Objective To conduct data mining on the literature related to prescriptions containing Tianma(Gastrodia Rhizoma),explore the medication rules and potential new prescriptions of modern prescriptions containing Tianma(Gastrodia Rhizoma),and provide a scientific reference for the clinical rational application of Tianma(Gastrodia Rhizoma).Methods Taking China National Knowledge Infrastructure(CNKI)and WanFang Database as the data sources,the literature on prescriptions containing Tianma(Gastrodia Rhizoma)from the establishment of the databases to January 2024 was retrieved.A database of modern literature on prescriptions containing Tianma(Gastrodia Rhizoma)was established using Microsoft Excel 2021.IBM SPSS Modeler 18.0 and SPSS Statistics 26.0 software were employed to extract and sort out the information of the retrieved prescriptions,including indications,syndrome types,high-frequency drugs,drug efficacy,nature,flavor,meridian tropism,and dosage of Tianma(Gastrodia Rhizoma).Subsequently,data mining techniques such as association rule analysis and cluster analysis were performed on the sorted data to explore the potential correlation and distribution characteristics.Results A total of 351 valid prescriptions containing Tianma(Gastrodia Rhizoma)were collected,covering 54 types of indications.The high-frequency diseases/syndromes(frequency≥5)were ranked in the order of vertigo,apoplexy,headache,tremor syndrome,etc.There were 21 kinds of high-frequency drugs(frequency≥50 times),which were Tianma(Gastrodia Rhizoma),Tianma(Gastrodiae Rhizoma),Gancao(Glycyrrhizae Radix Et Rhizoma),Gouteng(Uncariae Ramulus Cum Uncis),Banxia(Pinelliae Rhizoma),Fuling(Poria),etc.,in descending order of frequency.In terms of drug efficacy categories,the main types were wind-calming and spasm-relieving medicines,Qi-tonifying medicines,blood-activating and menstruation-regulating medicines.Regarding drug nature,the frequency from high to low was neutral,warm,and cold;the drug flavors were mainly sweet,bitter,and acrid;and the meridian tropism was mainly liver meridian,spleen meridian,and lung meridian.The commonly used clinical dosage of Tianma(Gastrodia Rhizoma)was 10 g,while the maximum dosage could reach 50 g in the treatment of vertigo.Further analysis of the medication rules of Tianma(Gastrodia Rhizoma)in high-frequency diseases showed that:in the treatment of vertigo,Tianma(Gastrodia Rhizoma)mainly exerted the effect of suppressing hyperactive liver to calm wind;in the treatment of apoplexy,it focused on the effect of dispelling wind to relieve spasm;in the treatment of headache,it tended to the effect of dispelling wind to alleviate pain.Overall,the clinical application of Tianma(Gastrodia Rhizoma)did not deviate from the core connotation of"treating wind".Conclusion The compatibility and clinical application of modern prescriptions containing Tianma(Gastrodia Rhizoma)adhere to the core principle of"treating wind".Clinically,Tianma(Gastrodia Rhizoma)is often combined with wind-calming and spasm-relieving medicines,as well as Qi-tonifying and blood-activating medicines.The selected drugs are mostly neutral in nature,sweet,bitter,and acrid in flavor,and mainly tropic to the liver,spleen,and lung meridians.The recommended clinical dosage of Tianma(Gastrodia Rhizoma)is 10 g,with decoction as the main administration method,and the dosage can be appropriately adjusted according to different diseases and syndromes.The medication rules summarized in this study can provide valuable references for the research and development of new clinical prescriptions,as well as the standardized and rational use of Tianma(Gastrodia Rhizoma)in clinical practice.关键词
