TACE治疗肝细胞癌伴肺转移诱发远隔效应1例暨文献复习
TACE-Induced Abscopal Effect in Hepatocellular Carcinoma with Pulmo-nary Metastasis:A Case Report and Literature Review

Abstract
The abscopal effect describes the regression of tumors outside a localized treatment field following radiothera-py.Emerging evidence indicates that this phenomenon can also be induced by various other local therapies.Currently,the relationship between transarterial chemoembolization(TACE)for hepatocellular carcinoma(HCC)and the induction of the abscopal effect remains poorly understood.This report presents a case of HCC with lung metastases treated with TACE,in which an abscopal effect was induced and long-term survival was achieved,and also provides a literature review on the ab-scopal effect in HCC.The patient was a 39-year-old male diagnosed with HCC based on imaging and clinical findings.The disease was complicated by portal vein tumor thrombus with intrahepatic metastases,and bilateral lung metastases.The staging was as follows:T4NxM1(Stage Ⅳ)according to the TNM system;China Liver Cancer Staging(CNLC)stage Ⅲb;Barcelona Clinic Liver Cancer(BCLC)stage C.The portal vein tumor thrombus was classified as type Ⅲ according to Cheng's classification and as Vp4 according to the Japanese Liver Cancer Association(LCSGJ)classification system.After five TACE sessions,the intrahepatic lesions achieved a complete response according to mRECIST criteria,and an abscopal effect was observed,leading to regression of the bilateral lung me-tastases.During the 58-month follow-up period,no tumor recurrence or distant metastasis was detected.Combined with the literature review,it is suggested that TACE,as a local treatment,may also induce abscopal effects capable of controlling dis-tant tumors.关键词
