火针疗法"温通效应"对脊髓损伤后微循环功能的调控机制研究
Regulatory Mechanism Underlying the"Warming Effects"of Fire-acupuncture Therapy on Microcirculation Function after Spinal Cord Injury

Abstract
Objective:To explore the regulation and mechanism of the"warming effect"of fire-acupuncture(FA)therapy on mi-crocirculation function after spinal cord injury(SCI).Methods:Forty-eight female Sprague Dawley(SD)rats were randomly divid-ed into Control group,Sham operation group,Model group,and FA group using a random number table,with 12 rats in each group.The FA group received FA stimulation to the Governing Vessel and the Jiaji points once daily for a total of 14 days.Detection indi-cators included the general conditions of rats.The Basso,Beattie,Bresnahan Locomotor Rating Scale(BBB)score was used to as-sess the recovery degree of nerve function.Infrared thermometry imaging technology was used to observe the local temperature fluc-tuation.Laser speckle blood flow imaging system was used to detect the regional spinal cord blood flow status and blood perfusion.The blood-spinal cord barrier(BSCB)permeability in spinal cord tissue was detected by Evans blue(EB)staining.Western blot and qRT-PCR were used to detect the expression level of nucleotide-binding oligomerization domain-like receptor protein 3 inflam-masome in spinal cord tissue(NLRP3)protein and mRNA in spinal cord tissue.qRT-PCR was used to detect the expression level of Heat shock protein B1(HSPB1)mRNA in spinal cord tissue.The expression levels of serum interleukin-1 β(IL-1 β)and inter-leukin-18(IL-18)were detected by enzyme-linked immunosorbent assay(ELISA).Results:Compared with the Sham group,the Model group exhibited decreased body weight(P<0.05),reduced BBB score(P<0.01),diminished blood perfusion(P<0.01),increased EB dye leakage(P<0.01),elevated serum IL-1 β and IL-18 levels(P<0.01),and heightened spinal cord inflamma-tion.Compared with the Model group,the FA group showed increased BBB score(P<0.01),an upward trend in local body temper-ature,improved blood perfusion(P<0.05),reduced EB dye leakage(P<0.05),decreased serum IL-1 β and IL-18 levels(P<0.05),downregulated expression levels of NLRP3 gene and protein in spinal cord tissue(P<0.05),and upregulated expression levels of HSPB1 gene and protein(P<0.01).Conclusion:ZThe"warming effect"of FA therapy can regulate and protect micro-circulation function after SCI,and its possible mechanism involves activating the target factor HSPB1,inhibiting the activation of the NLRP3 inflammasome,reducing serum IL-1β and IL-18 levels,improving BSCB permeability,increasing blood perfusion,maintai-ning vascular homeostasis,and promoting nerve function repair.关键词
