自拟益气调脾解毒方对重症患者贫血的疗效观察
Observation on the efficacy of self-designed Yiqi Tiaopi Jiedu Decoction for anemia in critically ill patients

Abstract
Objective To observe the clinical efficacy of Yiqi Tiaopi Jiedu Decoction(Qi-Boosting,Spleen-Regulating,Toxin-Resolving Decoction)in treating anemia in critically ill patients with the pattern of Qi deficiency and toxin stagnation.Methods A total of 72 patients were selected from the Intensive Care Unit(ICU)and Intensive Rehabilitation Unit of the Ningxia Integrated Chinese and Western Medicine Hospital.These patients were hospitalized between January 2023 and December 2024 and met the inclusion criteria.Then,they were randomly divided into an observation group and a control group using a random number table,with 36 patients in each group.The control group received folic acid tablets(10mg,three times daily)and cobamamide tablets(1.0mg,three times daily).The observation group received Yiqi Tiaopi Jiedu Decoction in addition to the conventional Western medication given to the control group.Treatment lasted for 3 weeks in both groups.Changes in the following parameters were monitored before treatment and after 3 weeks of treatment:Red blood cell count(RBC),hemoglobin(HGB),hematocrit(HCT),total protein(TP),albumin(ALB),prealbumin(PA),cholinesterase(CHE),creatinine(CREA),blood urea nitrogen(BUN),and blood urea nitrogen/creatinine ratio(UCR).Using a quantitative TCM syndrome scoring system,the total scores for main symptoms and signs(sallow complexion,mental fatigue and weakness,shortness of breath with reluctance to speak,poor appetite,epigastric and abdominal distension,excessive phlegm,sticky stool,squamous and dry skin,pale-dark tongue,deep-choppy pulse)were compared between the two groups before and after treatment.Results The total effective rate was 80.56%in the observation group and 50%in the control group.The observation group was significantly superior to the control group,and the difference in total effective rate was statistically significant(P＜0.05).The TCM symptom and sign scores in the observation group were significantly better than those in the control group,with statistically significant differences(P＜0.05).After treatment,inter-group comparisons showed statistically significant differences between the two groups for HGB,TP,ALB,and CHE(P＜0.05).Conclusion Yiqi Tiaopi Jiedu Decoction demonstrates good efficacy in treating anemia in critically ill patients with the Qi deficiency and toxin stagnation pattern.It can alleviate main TCM symptoms and signs,elevate HGB levels to effectively improve anemia,while also increasing TP,ALB,and CHE levels to improve nutritional indicators.It is worthy of further promotion.关键词
