OLIF与微创经椎间孔入路腰椎融合术治疗单节段腰椎管狭窄症的疗效比较

Abstract
Objective To compare and analyze the clinical efficacy and radiographic outcomes of oblique lumbar interbody fusion(OLIF)and minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)in the treatment of single-level lumbar spinal stenosis(LSS).Methods A retrospective cohort study was used,which included 63 patients with LSS who strictly met the inclusion criteria at the East Branch of Qingdao Municipal Hospital between April 2021 and November 2023.Thirty-five patients were sub-jected to OLIF(OLIF group),and 28 patients were treated with MIS-TLIF(MIS-TLIF group).All patients were followed up for more than one year.Perioperative data,visual analogue scale(VAS)for leg and back pain,Oswestry disability index(ODI),and radiographic parameters were compared between the two groups.Results Compared with the MIS-TLIF group,the OLIF group demonstrated shorter operative time,less intrao-perative blood loss,and a shorter hospital stay(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Compared with preoperative values,VAS and ODI scores at all postoperative time points were significantly reduced in both groups(P<0.05).At 3 days postoperatively,the improvement in back pain VAS scores was significantly better in the OLIF group than in the MIS-TLIF group(P<0.05).Radiographic results showed that intervertebral height and lumbar lordosis angle improved signifi-cantly at all postoperative time points compared to preoperative values in both groups(P<0.05),with no signifi-cant differences between groups.The cross-sectional area of the spinal canal improved significantly at 3 days postoperatively after surgery in both groups(P<0.05),with no intergroup difference.The intraoperative bone graft area was larger in the OLIF group than in the MIS-TLIF group(P<0.05).The lumbar fusion rate at 6 months postoperatively was higher in the OLIF group(88.6%)than in the MIS-TLIF group(67.9%),a statistically significant difference(P<0.05).No significant difference in fusion rate was observed between the two groups at 12 months postoperatively.Conclusion For patients with single-level LSS,both OLIF and MIS-TLIF can safely achieve decompression,with comparable long-term fusion rates.OLIF offers advantages including less surgical trauma,faster pain relief,and a larger bone graft area translating into a better early fusion rate.关键词
