营养状况与喉癌患者同步放化疗期间治疗毒性的关联探究
Association between nutritional status and treatment toxicity during concurrent chemoradiotherapy in patients with laryngeal cancer

Abstract
Objective To investigate the association between nutritional status and treatment toxicity during concurrent chemoradiotherapy in patients with laryngeal cancer.Method A total of 120 patients with laryngeal cancer who underwent concurrent chemoradiotherapy in Hunan Provincial People's Hospital from January 2022 to January 2025 were enrolled.Based on the patient-generated subjective global assessment(PG-SGA)results,patients were divided into a well-nourished group and a malnourished group.The incidence of treatment toxicities during concurrent chemoradiotherapy was compared between the two groups.Multivariate Logistic regression analysis was used to identify factors associated with the occurrence of grade≥3 severe toxic reactions during concurrent chemoradiotherapy.Result Among the 120 patients,PG-SGA grades A,B,and C before concurrent chemoradiotherapy were identified in 42(35.00%),52(43.33%),and 26(21.67%)patients,respectively,with the proportion of malnourished patients being 65.00%.The incidences of grade≥3 radiation mucositis,dysphagia,myelosuppression,nausea/vomiting,infection/fever,and the rate of treatment interruption/delay due to toxicity in malnourished group were significantly higher than those in the well-nourished group(all P<0.05).After 4 cycles of treatment,the total lymphocyte count(TLC)decreased in both groups compared to pre-treatment levels(all P<0.05);body mass index(BMI),albumin(ALB),and hemoglobin decreased in the malnourished group after 4 cycles of treatment(all P<0.05),with a more significant reduction compared to the well-nourished group(all P<0.05).Among all patients,48(40.00%)experienced grade≥3 toxic reactions.Multivariate Logistic regression analysis indicated that advanced age,decreased BMI,TNM stage Ⅳ,TP chemotherapy regimen,and malnutrition were independent risk factors for grade≥3 treatment toxicity during concurrent chemoradiotherapy in laryngeal cancer patients(all P<0.05).Conclusion A high proportion of laryngeal cancer patients are malnourished before concurrent chemoradiotherapy.Malnourished patients exhibit higher incidences of grade≥3 toxic reactions and treatment interruptions/delays during therapy,along with a greater risk of nutritional status deterioration due to chemoradiotherapy.Additionally,grade≥3 toxic reactions are influenced by age,BMI,TNM stage,and chemotherapy regimen.关键词
