Impact of implantable collamer lens implantation on the ocular surface

Abstract
Objective To evaluate the objective indicators of dry eye in patients following implantable collamer lens(ICL)implantation and analyze the changing trends of these indicators during the early postoperative period.Methods A total of 68 patients who underwent ICL implantation at the Myopia Center of the Urumqi Aier Eye Hospital between March 2024 and March 2025 were enrolled.Based on their preoperative ocular surface status,the patients were divided into two groups:the dry eye group(36 cases,36 eyes)and the non-dry eye group(32 cases,32 eyes).The right eye of each patient was selected for the analysis.All patients completed a series of examinations before surgery,as well as at 1 week,1 and 3 months post-surgery,including assessments of visual acuity,intraocular pressure(IOP),and refraction.Tear secretion was measured using the SchirmerⅠtest(SⅠt),and corneal integrity was evaluated using corneal fluorescein sodium(CFS)staining.The OCULUS Ocular Surface Analyzer was employed to determine the non-invasive first tear film break-up time(NIFBUT),non-invasive average tear film break-up time(NIABUT),and tear meniscus height(TMH).Additionally,scoring was performed for meibomian gland secretion characteristics,meibomian gland orifice morphology,and meibomian gland loss.Results No statistically significant differences were observed in the changes in SⅠt,TMH,or meibomian gland area between the two groups when comparing preoperative and postoperative values(P>0.05).CFS scores were significantly higher 1 week postoperatively than preoperatively(P<0.05),but no significant differences were found between the preoperative period and 1 or 3 months postoperatively(P>0.05).NIFBUT and NIABUT were significantly lower at 1 week and 1 month postoperatively than at preoperatively(P<0.05),while no significant dif-ferences were noted between preoperatively and 3 months postoperatively(P>0.05).Meibomian gland secretion scores were signifi-cantly higher at 1 week and 1 month postoperatively than preoperatively(P<0.05),with no significant difference between preoperation and 3 months postoperatively.Meibomian gland orifice scores were significantly lower 1 week postoperatively than preoperatively(P<0.05),but no significant differences were observed between preoperative and 1 or 3 months postoperatively(P>0.05).Conclusion The early postoperative period following ICL surgery has a certain degree of impact on the ocular surface microenvironment and meibomian gland function,but has no effect on the meibomian gland structure.关键词
