川芎嗪对老年重症肺炎患者炎症因子和凝血功能及氧化应激的影响
Effects of ligustrazine on inflammatory factors,coagulation function and oxidative stress in elderly patients with severe pneumonia

Abstract
Objective To investigate the regulatory effects of Ligustrazine adjunctive therapy on inflammatory factors,coagulation function and oxidative stress in elderly patients with severe pneumonia.Methods A total of 80 elderly patients with severe pneumonia admitted to the department of intensive care unit of the First Attached Hospital of Hebei North University from September 2021 to June 2023 were selected as the research subjects.The patients were randomly divided into conventional treatment group and Ligustrazine intervention group with 40 cases in each group.Both groups received standardized comprehensive treatments such as anti-infection,oxygen therapy and respiratory support,the Ligustrazine intervention group was additionally treated with Ligustrazine injection combined therapy.After 14 days of treatment,the levels of oxygenation index(PaO2/FiO2),superoxide dismutase(SOD),inflammatory mediators[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and procalcitonin(PCT)],and coagulation function parameters[prothrombin time(PT),activated partial thromboplastin time(APTT),and D-dimer]were observed in both groups.Meanwhile,the safety of the drugs and the tolerance of the patients during the entire treatment process were monitored.Results Before treatment,there was no statistically significant difference in each index between the two groups.After 14 days of treatment,the PaO2/FiO2,SOD activity,PT and APTT in both groups were significantly higher than before treatment,and the levels of IL-6,TNF-α,PCT,and D-dimer were significantly lower than those before treatment(all P<0.05).Moreover,after 14 days of treatment,the PaO2/FiO2,SOD activity,PT and APTT in Ligustrazine intervention group were significantly higher than those in the conventional treatment group[PaO2/FiO2(mmHg,1 mmHg≈0.133 kPa):389.12±25.56 vs.321.87±22.81,SOD(kU/L):127.15±11.72 vs.95.28±7.94,PT(s):13.71±0.96 vs.11.52±0.95,APTT(s):37.35±2.25 vs.32.57±2.72,all P<0.05],and the levels of IL-6,TNF-α,PCT and D-dimer were significantly lower than in the conventional treatment group[IL-6(ng/L):20.11±8.41 vs.58.32±11.31,TNF-α(ng/L):75.27±15.84 vs.104.76±13.65,PCT(ng/L):2.19±1.15 vs.4.34±1.78,D-dimer(mg/L):1.77±0.54 vs.2.89±0.75,all P<0.05].During the treatment period,both groups showed good tolerance and no drug-related toxic reactions were observed.Conclusions Ligustrazine can effectively inhibit inflammation in elderly patients with severe pneumonia,reduce oxidative stress response,increase pulmonary oxygenation,improve coagulation function,without increasing adverse reactions,and can be used as a therapeutic strategy in elderly patients with severe pneumonia.关键词
