血清β淀粉样蛋白40与β淀粉样蛋白42水平在糖尿病肾病中的表达和诊断意义
Expression and diagnostic significance of serum amyloid β-protein 40 and amyloid β-protein 42 levels in diabetic nephropathy

Abstract
Objective To investigate the expression and diagnostic significance of serum amyloid β-protein 40(Aβ40)and Aβ42 levels in diabetic kidney disease(DKD).Methods From January 2018 to January 2020,87 patients with DKD admitted to Taizhou Hospital Traditional Chinese Medicine were collected as the observation group,and 78 healthy subjects undergoing physical examination in the same hospital during the same period were enrolled as the control group.Differences in Aβ40,Aβ42,and baseline data between the two groups were compared.The 87 DKD patients were further divided into five subgroups according to DKD stages(DKDⅠ-Ⅴ),differences in serum Aβ40 and Aβ42 levels and baseline characteristics among varying degrees of disease severity were compared.Multivariate Logistic regression analysis was used to screen for influencing factors of DKD.The diagnostic value of Aβ40 and Aβ42 levels for DKD was evaluated using receiver operator characteristic curve(ROC curve)analysis.Results Serum levels of Aβ40 and Aβ42 in observation group were higher than those in the control group[Aβ40(μg/L):113.71±5.96 vs.69.58±3.68,Aβ42(μg/L):91.47±6.54 vs.59.27±4.19,both P<0.05].Multivariate Logistic regression analysis showed that both Aβ40[odds ratio(OR)=1.130,95%confidence interval(95%CI)was 1.060-1.210,P<0.001]and Aβ42(OR=1.100,95%CI was 1.020-1.180,P=0.013)were influencing factors for the development of DKD.ROC curve analysis showed that both Aβ40 and Aβ42 had certain predictive value for the occurrence of DKD(both P<0.05).Area under the curve(AUC)were 0.940 and 0.880,respectively.When the optimal cut-off value of Aβ40 was 121 μg/L,its sensitivity was 89.4%and specificity was 73.6%;when the optimal cut-off value of Aβ42 was 113 μg/L,its sensitivity was 78.7%and specificity was 79.2%.The levels of Aβ40 and Aβ42 gradually increased with the severity of the disease(all P<0.05).Conclusion Serum levels of Aβ40 and Aβ42 are elevated in patients with DKD and increase progressively with disease severity,the serum Aβ40 and Aβ42 levels have high clinical value in the diagnosis of DKD and can provide guidance for the clinical treatment of DKD.关键词
