凝血功能指标和D-二聚体与早期甲状腺乳头状癌患者临床病理特征的关系分析
Association between coagulation function,D-dimer levels and clinicopathological characteristics of patients with early-stage papillary thyroid carcinoma

Abstract
Objective To analyze the relationship of preoperative coagulation function and D-dimer(D-D)levels with clinicopathological characteristics in patients with early-stage papillary thyroid carcinoma(PTC).Methods A retrospective study was conducted on 296 patients with newly diagnosed thyroid nodular diseases who underwent surgical treatment at Ningbo Beilun District People's Hospital between January 2020 and September 2023.Among them,219 patients had thyroid cancer(including 207 cases of early-stage PTC,7 cases of thyroid follicular carcinoma,and 5 cases of thyroid medullary carcinoma),and 77 patients had benign thyroid tumors.Blood samples were collected within 3 days before surgery to measure prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(Fib),and D-D levels.Clinicopathological data were collected,and the relationship of coagulation parameters and D-D levels with clinicopathological features was analyzed.Results No statistically significant differences were observed in PT,APTT,TT,Fib,or D-D levels between thyroid cancer patients and benign thyroid tumor patients(all P＞0.05).Among the above three types of thyroid cancer patients,no significant differences were observed in PT,APTT,TT,Fib,or D-D levels(all P＞0.05).In early-stage PTC patients,no significant differences were found in PT,APTT,or TT levels across patients with different ages,tumor diameters,TNM stage,multifocality,presence of Hashimoto's thyroiditis,capsular invasion,lymph node metastasis,or BRAF mutation status(all P＞0.05).However,patients aged ≥55 years had significantly higher Fib and D-D levels compared to those aged＜55 years(both P＜0.05).Conclusion Coagulation function indicators and D-D levels are not closely associated with major pathological features of early-stage PTC patients.However,patients aged ≥55 years with early-stage PTC should be closely monitored for Fib and D-D levels to prevent venous thrombosis.关键词
