超声引导下肩关节腔注射联合针刀松解术治疗肩关节周围炎
Ultrasound-Guided Intra-Articular Injection Combined with Needle Knife Release for Scapulohumeral Periarthritis

Abstract
Objective:To investigate the clinical efficacy and safety of intra-articular injection of dexamethasone palmi-tate combined with acupotomy adhesion release in the treatment of scapulohumeral periarthritis under ultrasound guid-ance,and to verify its clinical advantages as a minimally invasive integrated intervention.Methods:A retrospective anal-ysis of 60 patients with scapulohumeral periarthritis who were treated in Hubei Provincial Hospital of Traditional Chi-nese Medicine from January 2022 to December 2024 was divided into treatment group(ultrasound-guided intra-articu-lar injection combined with acupotomy and control group(simple ultrasound-guided injection),30 cases in each group.The changes of pain score(VAS),shoulder joint mobility,SF-36 quality of life score,Japanese Orthopaedic Association(JOA)score and simplified McGill pain questionnaire(SF-MPQ)score were compared between the two groups before and after treatment,and adverse reactions were recorded.Results:Two groups of patients were significantly improved(P<0.05),the treatment group in the VAS score(3.2±0.4 VS 4.5±0.6),shoulder flexion/abduction activity(142.3±6.2 VS 123.5±7.8;133.6±7.5 VS 113.2±8.7)and SF-36,JOA,SF-MPQ scores were better than the control group(P<0.01).No serious adverse reactions were observed.Conclusion:Ultrasound-guided intra-articular injection combined with acupotomy lysis is significantly superior to single treatment in relieving pain,restoring function and improving quality of life.It has the advantages of precision,minimally invasive,safety and repeatability.It is suit-able as an optimization scheme for the intervention of scapulohumeral periarthritis in the middle and late stage.关键词
