天癸散敷贴治疗2型糖尿病伴骨量减少的疗效及其对TGF-β1水平的影响
Efficacy of Tiangui Powder in the Treatment of Type 2 Diabetes with Osteopenia and Its Effect on TGF-β1 Level

Abstract
Objective:To investigate the therapeutic effect of Tiangui Powder on patients with type 2 diabetes mellitus(T2DM)complicated with reduced bone mass,and its influence on the level of transforming growth factor-β1(TGF-β1).Methods:A to-tal of 65 patients diagnosed with T2DM and reduced bone mass were enrolled and randomly assigned into the Tiangui Powder and the basic treatment group using stratified block randomization,with 30 cases in each group after 5 dropouts.The Tiangui Powder received standard treatment(hypoglycemic therapy,calcium,and vitamin D supplementation)combined with acupoint application of Tiangui Powder at Shenque(CV8),Shenshu(BL23),Guanyuan(CV4),and Zusanli(ST36),once daily for 6 hours each time,over a course of 3 months.The basic treatment group received only standard treatment and placebo applica-tion.Before and after treatment,lumbar bone mineral density(BMD),visual analogue scale(VAS)score,glucose metabolism indicators(fasting blood glucose[FBG],glycated hemoglobin[HbA1c]),bone metabolic markers(alkaline phosphatase[ALP],procollagen type I N-terminal propeptide[P1NP],N-mid osteocalcin[N-MID],β-C-terminal telopeptide of type I col-lagen[β-CTX]),and serum TGF-β1 levels were assessed.Clinical efficacy was evaluated based on comprehensive efficacy cri-teria.Results:After treatment,the VAS score in the Tiangui Powder was significantly lower than that in the basic treatment group(t=-2.55,P=0.014).Improvements in FBG and HbA1c were also superior in the Tiangui Powder.The levels of bone formation markers P1NP and N-MID significantly increased,and the serum TGF-β1 level was markedly elevated com-pared with pre-treatment levels.Conclusion:Acupoint application of Tiangui Powder can alleviate pain and improve glucose metabolism in patients with T2DM and reduced bone mass,potentially by modulating TGF-β1 levels and promoting bone me-tabolism.关键词
