定量光激发荧光技术辅助口腔龋病的检测与评估效果研究
Research on the effect of quantitative light-induced fluorescence technology on the detection and assessment of dental caries

Abstract
Objective To investigate the role of quantita-tive light-induced fluorescence(QLF)technology in as-sisting the detection and assessment of dental caries.Methods From July 2024 to March 2025,a total of 120 extracted permanent molars and premolars(140 pit and fissure carious lesions)from the Department of Oral and Maxillofacial Surgery,Peking University School and Hos-pital of Stomatology,were included.All samples were scanned using the TRIOS 5 intraoral scanner.Conventional color-textured 3D models and fluorescence-enhanced 3D models with dual-modality textures(color and fluorescence)were obtained through QLF-assisted intraoral scanning.The carious lesions on the physical specimens,conventional color-textured models,and fluorescence-enhanced models were graded according to the International Caries Detection and Assessment System(ICDAS),which was used as the diagnos-tic reference standard.Sensitivity and specificity were calculated for each ICDAS level(0-6)to evaluate detection per-formance.Weighted Kappa coefficients were used to assess ordinal consistency between each 3D model and ICDAS grad-ing,with Kappa>0.8 indicating diagnostic consistency,and Kappa differences were compared between the two 3D mod-els.Results The Kappa values of fluorescence-enhanced and conventional color-textured 3D models were 0.811(95%CI:0.738-0.886)and 0.525(95%CI:0.431-0.619),respectively,showing a statistically significant difference(Z=4.686,P<0.001).For fluorescence-enhanced models,the sensitivity and specificity for detecting caries of differ-ent ICDAS grades were as follows:0.846 and 0.969 for grade 0,0.762 and 0.958 for grade 1,0.909 and 0.975 for grade 2,0.744 and 0.970 for grade 3,0.846 and 0.945 for grade 4,1.000 and 1.000 for grade 5,and 0.926 and 0.983 for grade 6.Conclusion Quantitative light-induced fluorescence technology can assist in the detection and assessment of dental caries,whose results show high consistency with ICDAS grading and demonstrate significant advantages over convention-al intraoral scanning.关键词
