糖皮质激素和阿达木单抗减量治疗对复发性VKH综合征患者疗效及炎性体征的影响

Abstract
Objective To observe the clinical effect of adalimumab combined with hormone reduction in the treat-ment of Vogt-Koyanagi-Harada(VKH)syndrome.Methods Retrospective case study.25 patients(48 eyes)with VKH syndrome who were treated with drug control for 3 months and the Adalimumab(ADA)course was gradually extended un-til inflammation developed and then returned to the initial treatment interval from February 2020 to October 2023 were col-lected.Anterior chamber cell(ACC)grade,vitreous opacity,retinal/choroiditis,serous retinal detachment(SRD),better corrected visual acuity(BCVA),central macular thickness(CMT),glucocorticoid and immunosuppressive dosage were observed through 12 mo follow-up.Treatment failure and side effects were observed.Results Compared with baseline,at 2 weeks,1 month,3 months and 6 months after the treatment the rate of eye disease increased(P﹤0.05),accompanied by a decrease in retinal/choroiditis(P﹤0.01),BCVA and CMT(P﹤0.01).The use of glucocorticoids decreased significantly(P﹤0.01).Conclusions ADA has a good therapeutic effect on VKH syndrome,which can reduce the use of glucocorti-coids and immunosuppressants,prolong the drug use cycle and reduce the recurrence rate.关键词
