Expression of ERCC1 in Breast Cancer Tissue based on Immunohistochemical Analysis and its Role in Postoperative Distant Metastasis

Abstract
Objective This study investigated the expression of the DNA repair gene resection and repair of cross-complementary group 1(ERCC1)in breast cancer tissues and its impact on postoperative distant metastasis,aiming to provide insights for prognosis assessment and therapeutic target selection in breast cancer patients.Methods A prospective nested case-control study was conducted using a breast cancer cohort established in 2013 at the Affiliated Hospital of Southwest Medical University.Cases were selected and matched 1:1 with controls based on key treatment parameters including patient age,surgery date,surgical approach,and chemotherapy regimen.Participants were divided into a metastatic group(n=93)and a non-metastatic group(n=93)according to the occurrence of distant metastasis post-surgery.Immunohistochemical analysis assessed ERCC1 expression differences between meta-static and non-metastatic groups.ROC curves were generated using R software to determine optimal cutoff values.Cox proportional haz-ards regression analysis via SPSS software explored ERCC1's role in predicting postoperative metastasis.Results There were statisti-cally significant differences between the two groups of breast cancer patients in progesterone receptor status and lymph node metastasis count(P<0.05),while other clinical pathological characteristics showed no statistical significance(P>0.05).ERCC1 was predomi-nantly localized in the nuclei of breast cancer cells,with positive expression appearing as brownish-yellow granules.Protein expression of ERCC1 was higher in the metastatic group than in the control group(P<0.05).ERCC1 was identified as a risk factor for breast cancer metastasis(HR=1.155,95%CI:1.007~1.323,P=0.039).The optimal cutoff value on the ROC curve was 1.5,dividing patients into high-and low-expression groups.The high-expression group exhibited a 5.327-fold increased risk of metastasis com-pared to the low-expression group(HR=5.327,95%CI:1.894~14.988,P=0.002).Conclusion ERCC1 is a risk factor for postop-erative metastasis in breast cancer patients and may serve as a potential biomarker for prognosis assessment and a therapeutic target.关键词
