烧伤康复期患者自我隐瞒与社交回避及苦恼的交叉滞后分析
Cross-Lagged Analysis of Self-concealment and Social Avoidance with Distress in Patients Recovering from Burns

Abstract
Objective To explore the trends of self-concealment and social avoidance with distress in patients recovering from burns and the predictive relationship between them,aiming to provide a basis for breaking the psychosocial vicious cycle and formulating sequential interventions.Methods From January 2021 to January 2024,280 patients recovering from burns admitted to the Affiliated Hospital of Jiujiang University were selected by convenience sampling.They were investigated using the Self-Concealment Scale(SCS)and the Social Avoidance and Distress Scale(SADS)at T1(when their conditions were stable as assessed by a physician after admission),T2(3 months after T1)and T3(6 months after T1).A cross-lagged model was constructed to analyze the trends of self-concealment and social avoidance with distress and the bidirectional predictive relationship between them.Results Repeated measures analysis of variance(ANOVA)revealed significant time effects on SCS scores[T1:(44.30±2.69)points,T2:(30.14±3.20)points,T3:(16.41±3.48)points]and SADS scores[T1:(25.89±2.37)points,T2:(18.49±3.19)points,T3:(10.25±3.59)points](P<0.05).Both scores decreased over time(T1>T2>T3).Pearson correlation analysis revealed a positive correlation between SCS and SADS scores across all three time points(P<0.05).The cross-lagged model showed that SCS at T1 positively predicted SADS at T2(β=0.143,P<0.05);SADS at T1 positively predicted SCS at T2(β=0.164,P<0.05);SCS at T2 positively predicted SADS at T3(β=0.155,P<0.05);SADS at T2 positively predicted SCS at T3(β=0.237,P<0.05).Conclusion The levels of self-concealment and social avoidance with distress decrease over time in patients recovering from burns and exhibit a bidirectional positive predictive relationship across time points.Clinically,psychological intervention should be emphasized for such patients to help reduce self-concealment and improve social avoidance and distress,thereby promoting full recovery.关键词
