阴火病机与升阳散火汤的临床应用探讨

Abstract
LI Dongyuan's concept of"yin fire"refers to a type of internally generated pathogenic heat,with"yin"denoting"internal."This condition originates from impairment of the spleen and stomach due to internal damage from the seven emotions,irregular diet,or overexertion.This,in turn,leads to spleen and stomach deficiency,sinking of qi,and a failure of clear yang to ascend.Consequently,the yang qi of the middle energizer becomes stagnant internally and transforms into heat.Clinically,yin fire may manifest as unre-strained heart fire,internally generated damp-heat,upward-flaring ministerial fire,among other presentations.Despite their variety,these manifestations share a common root in spleen and stomach deficiency,qi sinking,and stagnant middle energizer yang qi that fails to diffuse and thus transforms into heat.Shengyang Sanhuo Decoction(升阳散火汤,SYSHD)is formulated to address this very condi-tion.Its therapeutic strategy integrates strengthening the spleen,tonifying qi,raising yang,and dispersing stagnated heat,thereby embodying the principles of"dispersing depressed fire"and"draining earth stagnation."Clinically,it is indicated for the pattern of spleen and stomach deficiency with internal stagnation of yang qi,frequently accompanied by liver qi stagnation.The clinical presenta-tion is characterized by fatigue,heaviness of the limbs,poor appetite,loose stools,insomnia,depression and discomfort,as well as subjective sensations of heat or intermittent fever(with or without measurable fever).Successful application hinges on understanding the core pathogenesis:spleen and stomach injury as the root cause and stagnated fire as the secondary branch.Flexible combination with dampness-eliminating,blood-invigorating,or yang-warming medicinals may be employed to achieve simultaneous treatment of both the root and the branch.关键词
