Abstract
Objective To observe the application value of quantitative bacterial count combined with urinary sediment white blood cell test in the diagnosis of urinary tract infection. Methods A total of 100 patients with suspected urinary tract infection who were treated in our hospital from June to October 2024 were selected as observation objects. All cases received simultaneous bacterial quantitative count and urine sediment white blood cell detection. The results of urine bacterial culture were used as the reference standard, and the results were statistically analyzed. In addition, the positive rate and diagnostic efficiency of different test methods, the consistency between the diagnostic accuracy of different test methods for urinary tract infection and the results of urine bacterial culture were analyzed, and the ROC curve was used to analyze the diagnosis of urinary tract infection by different test methods. Results The results of urine bacterial culture showed that 83 cases were positive in 100 samples, with a positive rate of 83.00%.The main pathogenic bacteria were Escherichia coli, followed by Streptococcus and Staphylococcus. The positive rate of bacterial quantitative count+urine sediment white blood cell detection was 81.00%,which was higher than 74.00%of bacterial quantitative count and 69.00%of urine sediment white blood cell test(P<0.05),however,no statistically significant difference was observed in the positive rates of the two methods(P>0.05).Compared with single detection, the combined application of bacterial quantitative count and urinary sediment white blood cell test showed advantages in various diagnostic efficacy indexes(including sensitivity, specificity, positive and negative predictive values),and the difference was statistically significant(P<0.05).The results of bacterial quantitative count+urine sediment white blood cell test were consistent with those of urine culture.ROC analysis revealed that the AUC for urinary sediment WBC count was 0.847,for bacterial quantitative count was 0.892,and for the combined test reached 0.965.Conclusion In the diagnosis of urinary tract infection, the combined detection of bacterial quantitative count and urinary sediment white blood cell has good performance in sensitivity and specificity, and has high consistency with urinary bacterial culture.关键词
