CHEN Xiangdong´s Experience in Treating Wet Age-related Macular Degeneration Based on Spleen-stomach-eye Xuanfu Theory
摘要
Abstract
Wet age-related macular degeneration(AMD)is a common ocular disease.Patients typically present with symptoms such as decreased visual acuity,metamorphopsia,dim vision,and central scotoma.Western Medicine primarily treats this condition with intravitreal injections of anti-vascular endothelial growth factor(VEGF)agents,which work by inhibiting VEGF.However,long-term efficacy is unsatisfactory and recurrence is frequent.The pathogenesis of this disease is related to imbalances in liver,spleen,and kidney.Traditional Chinese Medicine(TCM)demonstrates significant therapeutic efficacy.The Spleen-stomach-eye Xuanfu Theory originates from LI Dongyuan´s"Spleen-stomach Theory"and LIU Wansu´s"Xuanfu Theory"from the Jin and Yuan dynasties.This paper elevates this theory,further elucidating the physiological relationship between Spleen-stomach-eye Xuanfu Theory and the pathogenesis of wet AMD.CHEN Xiangdong proposes that the pathogenesis of wet AMD is rooted in spleen-stomach deficiency with Yin fire and damp-heat assailing the eyes,leading to eye Xuanfu obstruction as the secondary manifestation.He establishes the treatment principles of tonifying spleen-stomach,elevating Yang,dispersing fire,clearing heat,and dispelling dampness to open eye Xuanfu.Modified Chonghe Yangwei Decoction from Shen Shi Yao Han is applied with clinical variations,providing new perspectives and methods for the clinical management of this disease.关键词
