Exploring Diabetic Retinopathy Based on the Theories of"Qi Collateral"and"Blood Collateral"

Abstract
Diabetic retinopathy(DR),a common complication of diabetes,has an increasing incidence year by year and seriously endangers patients´ visual health.Based on the theories of"Qi collateral"and"blood collateral",this paper systematically analyzes the pathological mechanism and treatment strategies of DR.Modern medicine holds that retinal microangiopathy induced by long-term hyperglycemia is the core cause of retinal ischemia,hypoxia and neovascularization.The pathogenesis of DR is essentially a deficiency in origin and excess in superficiality,with Qi and Yin deficiency as the root cause and intermingling of phlegm and blood stasis as the superficial symptom.Its pathogenesis evolution is reflected in the dynamic changes of Qi collateral and blood collateral.Derived from the collateral disease theory,the theories of"Qi collateral"and"blood collateral"can explain the pathogenesis rules of DR in different stages by combining the characteristics of retinal micro-lesions:Qi collateral stagnation is the main manifestation in the early stage,blood stasis and phlegm in blood collateral are the key in the middle and late stages,and Yin deficiency runs through the whole course of the disease.On this basis,the collateral dredging therapy with"collaterals being unobstructed as the fundamental function"becomes the key to the treatment of DR,which needs to combine specific therapies such as nourishing Yin,tonifying Qi and promoting Qi movement,nourishing blood and removing blood stasis,eliminating phlegm and dredging collaterals,while taking holistic syndrome differentiation into account.This study aims to enrich the Traditional Chinese Medicine(TCM)syndrome differentiation thinking of DR and provide theoretical support and practical guidance for improving the clinical efficacy of TCM in the treatment of DR.关键词
