Exploration on the Role of Autophagy Bidirectional Regulation in Rheumatoid Arthritis and the Intervention Mechanism of TCM Based on the Theory of"Dynamic Balance of Yin and Yang"
摘要
Abstract
The pathological process of rheumatoid arthritis(RA)is closely related to the imbalance of autophagy function.This article,starting from the theory of"dynamic balance of yin and yang"in TCM,explored the role of autophagy in different pathological stages of RA:excessive activation of autophagy,manifested as"positive"pathological function hyperactivity and relatively insufficient"negative"protective function,that is,"excess of yang and deficiency of yin",aggravates inflammation and bone destruction;while low autophagy function,manifested as"negative"clearance function failure and insufficient"positive"repair power,that is,"excess of yin and deficiency of yang",hinders tissue repair.TCM targets key autophagy pathways such as PI3K/AKT/mTOR to bidirectionally regulate autophagy activity,achieving"clearing heat and promoting diuresis"and"tonifying liver and kidney"and other staged treatment strategies.Monomers such as salvianolic acid B and formulas such as Danggui Niantong Decoction and Cheng's Juanbi Decoction significantly inhibit abnormal proliferation of synovial fibroblasts or promote cartilage repair through multi-target effects.By integrating theories of traditional Chinese and Western medicine,it can provide reference ideas for the diagnosis and treatment of RA and theoretical references for the development of new drugs based on autophagy regulation.关键词
