Efficacy of Glycine Subgingival Air Polishing in Patients with Type 2 Diabetes Mellitus and Periodontitis and Its Effects on Plaque Index and Inflammatory Factor Levels in Gingival Crevicular Fluid

Abstract
Objective To investigate the efficacy of glycine subgingival air polishing in the treatment of patients with type 2 diabetes mellitus(T2DM)and periodontitis and its effects on plaque index(PLI)and the levels of inflammatory factors in gingival crevicular fluid.Methods From June 2023 to December 2024,80 patients with T2DM and periodontitis diagnosed and treated at the Third Hospital of Hebei Medical University were selected and divided into a conventional group and an air polishing group using a simple randomization method(random number table),with 40 cases in each group.The conventional group received traditional ultrasonic scaling combined with manual subgingival scaling and root planing,while the air polishing group received glycine subgingival air polishing.Taking 1 month after initial periodontal therapy as the baseline,bleeding index(BI),probing depth(PD),PLI,and masticatory function(bite force,masticatory efficiency)were compared between the two groups at baseline and at 1,3,and 6 months of maintenance therapy.Glycemic and lipid parameters[glycated hemoglobin(HbA1c),triglyceride(TG),total cholesterol(TC),fasting blood glucose(FPG)]and gingival crevicular fluid inflammatory factors[tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-6(IL-6)]were compared at baseline and at 1 month of maintenance therapy.Results Compared with baseline,BI,PD,and PLI in both groups decreased significantly at 1,3,and 6 months of maintenance therapy(P<0.05),and all were significantly lower in the air polishing group than in the conventional group(P<0.05).Compared with baseline,bite force and masticatory efficiency in both groups increased significantly at 1,3,and 6 months of maintenance therapy(P<0.05),and both were significantly higher in the air polishing group than in the conventional group(P<0.05).At 1 month of maintenance therapy,the levels of HbA1c,TG,TC,FPG,TNF-α,CRP,and IL-6 decreased significantly in both groups(P<0.05),and all were lower in the air polishing group than in the conventional group(P<0.05).Conclusion Glycine subgingival air polishing can effectively treat periodontitis in patients with T2DM,improve periodontal health and masticatory function,reduce blood glucose and lipid levels,and alleviate gingival inflammatory response.关键词
