火针扬刺法联合麦粒灸治疗带状疱疹后遗神经痛的临床研究
Clinical Study of Centro-Square Fire Needling Combined with Wheat Grain-Sized Cone Moxibustion in Treatment of PHN

Abstract
Objective:To observe the clinical effect of centro-square fire needling combined with wheat grain-sized cone moxibustion in treatment of postherpetic neuralgia(PHN).Methods:A total of 104 patients with PHN were randomly divided into the study group and the control group,with 52 cases in each group.Both groups received conventional Western medical treatment.On this basis,the control group was additionally treated with wheat grain-sized cone moxibustion,and the study group received centro-square fire needling in addition to the treatment given to the control group.The scores of TCM symptoms,VAS,and PSQI,as well as the serum levels of endorphin(EP),substance P(SP),calcitonin gene-related peptide(CGRP),monocyte chemoattractant protein-1(MCP-1),interferon-inducible protein-10(IP-10)and fractalkine(FKN)were compared between before and after the treatment in the two groups.Results:The total effective rate was 92.31％(48/52)in the study group,which was higher than 82.69％(43/52)in the control group,but there was no statistical difference between the two groups(P＞0.05).The scores of TCM symptoms,VAS,and PSQI,as well as the serum levels of EP,SP,CGRP,MCP-1,IP-10,and FKN were all significantly decreased after the treatment compared to those before the treatment in the two groups(P＜0.05);the decreases in the study group were more significant than those in the control group(P＜0.05).Conclusion:The combination of centro-square fire needling and wheat grain-sized cone moxibustion produces significant clinical efficacy in treatment of postherpetic neuralgia(PHN).This approach can effectively alleviate pain intensity and improve sleep quality in patients.Its therapeutic mechanism may be related to the regulation of pain-related substances and chemokine expression,suggesting that it is worthy of further clinical promotion and application.关键词
