慢性阻塞性肺疾病患者心肺耐力轨迹及影响因素研究
A study on the cardiorespiratory endurance trajectories and influencing factors in patients with chronic obstructive pulmonary disease

Abstract
Objective To explore the potential latent classes and influencing factors of cardiorespiratory endurance change trajectories in patients with chronic obstructive pulmonary disease(COPD).Methods Using convenience sampling,a total of 184 COPD patients hospitalized in the respiratory department of a tertiary hospital in Huzhou,Zhejiang Province from August 2023 to February 2024 were selected as the study subjects.General data were collected one day before discharge,un-planned readmission status was collected six months after discharge,and cardiorespiratory endurance indicators were meas-ured one day before discharge as well as one,three,and six months after discharge.Latent class growth analysis was applied to classify the trajectories of cardiorespiratory endurance indicators.Univariate analysis and logistic regression analysis were used to determine the impact of related variables on the trajectories.Results Three developmental trajectories of cardiores-piratory endurance were identified:a medium-level slow-rising group(91 cases,49%),a low-level slow-declining group(40 cases,22%),and a high-level slow-rising group(53 cases,29%).Gender,smoking status,lung function grade,Modified Medical Research Council Dyspnea Scale(mMRC),and COPD Assessment Test(CAT)score entered the regression equa-tion,with statistically significant differences(P<0.05).Conclusion There is population heterogeneity in the developmental trajectories of cardiorespiratory endurance among COPD patients,which is influenced by gender,smoking status,lung func-tion grade,dyspnea symptoms,and health status.关键词
