不同阈值算法下PET/CT代谢参数在非小细胞肺癌病理分期评估中的价值
Value of PET/CT Metabolic Parameters under Different Threshold Algo-rithms in Assessing Pathological Staging of Non-Small Cell Lung Cancer

Abstract
Objective:To evaluate the value of PET/CT metabolic parameters under different threshold algorithms for e-valuating the pathological staging of non-small cell lung cancer.Methods:Using a retrospective study design,100 patients with non-small cell lung cancer treated in our hospital from January 2023 to December 2024 were selected,including 34 pa-tients with pathological stages I-IIIa and 66 with stage IIIb.Clinical data and PET/CT metabolic parameters were compared between patients with different pathological stages.The metabolic parameters were derived using different threshold algo-rithms,including standardized uptake value thresholds of 2.5 and 4.0,as well as metabolic tumor volume(MTV)and total lesion glycolysis(TLG)at 40%and 50%of the maximum SUV.Results:There were no statistically significant differences between patients in stages I-IIIa and IIIb regarding gender,age,pathological type,tumor location,or tumor diameter(all P>0.05).The MTV2.5,TLG2.5,MTV4.0,TLG4.0,MTV40%,TLG40%,MTV40%,and TLG50%values for patients in stage IIIb were 32.50(22.80,65.54)cm3,165.50(52.70,332.25),20.05(8.89,35.50)cm3,120.20(60.40,168.87),18.89(9.90,32.50)cm3,130.40(48.80,150.50),12.50(6.80,20.00)cm3,and 105.57(55.50,134.40),respectively,which were significantly higher than those for patients in stages I~IIIa(P<0.05).No significant correlation was found between PET/CT metabolic parameters and patient characteristics with respect to gender,age,pathological type,tumor location,or tumor diameter(all P>0.05).The AUC of MTV2.5,TLG2.5,MTV4.0,TLG4.0,MTV40%,TLG40%,MTV50%and TLG50%for predicting pathological stage IIIb were 0.670,0.915,0.697,0.702,0.770,0.679,0.703,and 0.751,respectively.Conclusion:PET/CT metabolic parameters under different threshold algorithms are closely associated with the pathological staging of non-small cell lung cancer,with TLG2.5 showing the greatest predictive value for stages IIIb-IV.关键词
