多层螺旋CT灌注参数联合血清IL-6、SAA对慢性阻塞性肺疾病患者肺部感染的诊断价值
The diagnostic value of multi-slice spiral CT perfusion parameters combined with serum IL-6 and SAA for pulmonary infection in patients with chronic obstructive pulmonary disease

Abstract
Objective:To explore the value of multislice spiral CT combined with serum interleukin(IL)-6 and serum amyloid A(SAA)in the diagnosis of pulmonary infection in patients with chronic obstructive pulmonary disease(COPD).Meth-ods:A retrospective selection was made of 178 patients with COPD and suspected pulmonary infection admitted to our hospi-tal from January 2021 to March 2024.According to the sputum culture results,they were divided into the infection group(n=72)and the non-infection group(n=106),and 16-multislice spiral CT plain scan,perfusion scan and IL-6,SAA detection were performed.The 16-multislice spiral CT parameters(blood volume(BV),blood flow(BF)),serum IL-6,and SAA levels of the two groups of patients were compared,and the diagnostic value of BV,BF combined with serum IL-6 and SAA was analyzed.Results:The BV,IL-6 and SAA levels in the infection group were higher than those in the non-infection group,and the BF level was lower than that in the non-infection group.The differences were all statistically significant(P<0.05).The results of the ROC curve showed that the AUC values of BV,BF,IL-6 and SAA levels for diagnosing pulmonary infection in patients with COPD were 0.644,0.663,0.707 and 0.700,respectively.When the above indicators are combined,the diagnostic efficacy is the highest.At this time,the AUC value is 0.825,the sensitivity is 76.4%,and the specificity is 75.0%(P<0.05).Conclu-sion:Multislice spiral CT combined with serum IL-6 and SAA has a high value in the diagnosis of pulmonary infection in patients with COPD.关键词
