Effect of aquatic exercise on lower limb osteoarthritis:a meta-analysis

Abstract
AIM To systematically evaluate the efficacy of aquatic exercise for patients with lower limb osteoarthritis(OA)using meta-analysis method.METHODS Relevant literature was retrieved from PubMed,Cochrane Library,Embase,and Web of Science databases from their inception to June 2025,focusing on randomized controlled trials(RCT)assessing the efficacy of aquatic exercise for lower limb OA patients.Participants were assigned to either a control group(no specific intervention)or an aquatic exercise group(receiving aquatic exercise training).Meta-analysis was performed using RevMan 5.4,with the standardized mean difference(SMD)and 95%confidence interval(CI)used to assess the efficacy of aquatic exercise for OA.RESULTS Twenty-four RCTs involving 1 823 patients were ultimately included.Short-term follow-up results indicated that compared with the control group,the aquatic exercise group demonstrated significant improvements in joint pain(SMD=-0.75,95%CI:-1.03,-0.46,P<0.05),physical function(SMD=-0.62,95%CI:-0.94,-0.30,P<0.05],stiffness(SMD=-0.42,95%CI:-0.74,-0.10,P<0.05),and function in sport and recreation(SMD=-0.99,95%CI:-1.84,-0.14,P<0.05).Analysis restricted to patients with knee osteoarthritis also demonstrated that aquatic exercise improved all four clinical dimensions of OA.Mid-term follow-up results showed improvements in pain,physical function,and exercise and recreation function.Long-term follow-up results indicated enhanced functional outcomes in the pain dimension.CONCLUSION Aquatic exercise effectively improves pain,function,physical symptoms,and quality of life in patients with lower limb OA.关键词
