CT细胞外体积分数在急性胰腺炎诊断及分度中的应用
Application of CT extracellular volume fraction in diagnosis and grading of acute pancreatitis

Abstract
Objective:To investigate the application value of CT-derived extracellular volume fraction(ECV)in the diagnosis and grading of acute pancreatitis(AP).Methods:A total of 132 patients with AP were retrospectively enrolled as the AP group,and 132 patients who underwent contrast-enhanced abdominal CT for non-pancreatic diseases during the same period were selected as the control group.According to CT severity index(CTSI),the AP group was divided into the mild AP group(61 cases)and the moderate-to-severe AP group(71 cases).In all patients,the CT value of the pancreatic parenchyma on unenhanced CT(CTpre pancreas value)and that on the equilibrium phase of contrast-enhanced CT(CTpost pancreas value),as well as the CT value of the abdominal aorta at the same stage(CTpre aorta and CTpost aorta values)were measured.ECV,pancreatic enhancement ratio(PER)and absolute CT enhancement value(ΔHU)were then calculated.The above parameters were compared between the control group and the AP group,and between the mild AP group and the moderate-to-severe AP group.ROC curve analysis was further performed to evaluate the diagnostic performance of the parameters with significant differences.Results:CTpre pancreas value in AP group was significantly lower than that in the control group(P＜0.05),whereas CTpost pancreas value,ΔHU,PER and ECV in AP group were all higher than those in the control group(all P＜0.05).Between the mild AP group and the moderate-to-severe AP group,only ECV showed a statistically significant difference(P＜0.05).ROC curve analysis showed that ECV had the highest AUC for diagnosing AP.Conclusions:ECV has good diagnostic performance in the diagnosis of AP and has certain application value in distinguishing mild AP from moderate-to-severe AP.关键词
