低磷饮食管理联合渐进性抗阻训练在维持性透析中的应用
Application of low-phosphorus diet management combined with progressive resistance training in patients undergoing ma-intenance hemodialysis

Abstract
Objective To explore the effects of low-phosphorus diet management combined with progressive resistance training on cardiac function and quality of life in maintenance hemodialysis(MHD)patients.Methods A total of 86 MHD patients treated in the hemodialysis unit of Changping District hospital from March 2024 to March 2025 were randomly assigned to either an observa-tion group or a control group,with 43 patients in each group.The control group received routine nursing care,while the observation group additionally received structured low-phosphorus diet management and progressive resistance training.Both groups underwent continuous intervention for 6 months.Cardiac function indicators,quality of life scores,and exercise tolerance were compared be-tween the two groups.Results After the intervention,the observation group showed significantly higher levels of hemoglobin[(112.88±8.92)g/L vs(106.86±12.00)g/L],serum calcium[2.49(2.43,2.54)mmol/L vs 2.12(2.05,2.15)mmol/L],and albumin[(42.31±0.73)g/L vs(41.10±0.71)g/L],and a lower level of serum phosphorus[(1.40±0.09)mmol/L vs(2.02±0.15)mmol/L]compared with the control group(all P<0.05).In addition,left ventricular end-diastolic diameter(LVEDD)[(47.02±0.53)mm vs(48.39±0.49)mm]and left ventricular end-systolic diameter(LVESD)[(39.37±0.85)mm vs(40.49±1.21)mm]were lower,while left ventricular ejection fraction(LVEF)[(52.37±1.17)%vs(51.07±1.26)%]was higher in the observation group(P<0.05).After 6 months,the observation group also demonstrated a longer 6-minute walking dis-tance,lower fatigue scores(FS-14),and higher quality of life scores(KDQOL-36)than the control group(all P<0.05).No signif-icant difference was observed in the incidence of adverse events between the two groups(P>0.05).Conclusion Combined low-phosphorus diet management and progressive resistance training can improve cardiac function and nutritional status,reduce fatigue,and enhance exercise tolerance and quality of life in MHD patients,supporting its clinical application.关键词
