急性缺血性脑卒中住院老年患者认知衰弱现状及影响因素研究
Study on the prevalence and influencing factors of cognitive frailty in hospitalized elderly patients with acute ischemic stroke

Abstract
Objective To investigate the prevalence and influencing factors of cognitive frailty in hospitalized elderly patients with acute ischemic stroke(AIS),and to provide a preliminary theoretical basis for health management and intervention in this popu-lation.Methods Patients aged≥60 years with AIS admitted to the Department of Geriatrics and Neurology,Qinzhou First People's Hospital from December 2022 to December 2023 were enrolled.Demographic,clinical,and laboratory data were collected.Risk factors were screened through univariate analysis and logistic regression,and a predictive model was constructed and validated.Results A total of 226 patients were included,with a cognitive frailty prevalence of 26.5%(60 cases).Univariate analysis showed significant associations between cognitive frailty and female gender,advanced age,low education level,unmarried status,hypertension,high NIH-SS and mRS scores,low ADL scores,elevated homocysteine(Hcy),increased neutrophiltolymphocyte ratio(NLR)and platelet-tolymphocyte ratio(PLR),and decreased lymphocyte and hemoglobin counts(P<0.05).Multivariate logistic regression identified age(OR=1.113),hypertension(OR=3.411),and elevated neutrophil count(OR=1.168)as independent risk factors,while a high ADL score(OR=0.970)was a protective factor(all P<0.05).ROC curve analysis for the prediction model:Regression equation:F=-8.636+0.107×age+1.227×hypertension(yes)-0.030×ADL score-0.155×neutrophil count.Prediction model:Y=ExP(F)/[1+ExP(F)].The area under the curve(AUC)was 0.806,with sensitivity 80.0%and specificity 75.9%.Conclusion The prevalence of cognitive frailty in hospitalized elderly AIS patients is high,with advanced age,hypertension,inflam-matory response,and low living ability as main influencing factors.Targeted health management and intervention are needed for this population.关键词
