经皮肝穿刺胆道镜碎石术联合引流管扩张术在肝胆管结石病中的疗效
Efficacy of percutaneous transhepatic cholioscopy lithotomy combined with stent dilation in the treatment of hepatolithiasis

Abstract
Objective To explore the efficacy of percutaneous transhepatic cholioscopy lithotomy(PTCSL)combined with stent dilation in the treatment of hepatolithiasis.Methods The clinical data of 17 patients with hepatolithiasis treated with PTCSL combined with stent dilation in the First People's Hospital of Chenzhou City from January 2024 to June 2024 were retrospectively analyzed.All patients underwent PTCSL to remove biliary stones and to dilate and stent the biliary stricture found during the operation.The related indexes during and after the operation,and the postoperative recovery was evaluated.Results Six cases underwent percutaneous transhepatic cholangial drainage(PTCD)or endoscopic retrograde cholangiopancreatiography(ERCP)drainage before PTCSL.In all 17 patients,the biliary stones were removed by PTCSL and stents were placed to expand the biliary stricture.Among them,14 cases underwent unilateral PTCSL,3 cases underwent bilateral PTCSL.No bleeding,infection,bile leakage,or other complications occurred postoperatively,and there was no death in the perioperative period or long-term complications.After a 1-year follow-up,no recurrence of stones was observed.Conclusion PTCSL combined with stent dilation has good safety,effectiveness,and repeatability in the treatment of hepatolithiasis and prevention of recurrence.In the case of difficulty in choledocholithotomy and unfeasible ERCP,PTCSL can be a preferred choice.关键词
