ALPPS for spontaneous rupture of hepatocellular carcinoma:A case report

Abstract
Associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)is a surgical method that could induce rapid future remnant liver(FLR)hypertrophy for patients with insufficient liver volume contemplating major hepatectomy.In this paper,we reported our experience of this approach for a 40-year-old patient with spontaneousl rupture of hepatocellular carcinoma(HCC)and intrahepatic metastasis.Computed tomography(CT)showed spontaneousl rupture of HCC(11.9 cm×11.3 cm×10.5 cm)at liver segment 6 and intrahepatic metastasis at liver segment 8(7.0 cm×6.0 cm)in the future remnant liver volume(FLRV)assessment of the left lobe was 29%of the estimated total liver volume(ESLV).ALPPS was performed in view of the small-for-size FLR.Due to ruptured segment 6 tumour in one-stage ALPPS,segment 6 wedge resection was performed.Two-stage ALPPS right hepatectomy was performed on day 7 after surgery when the FLRV increased to 37.78%ESLV.The patient recovered stably and was discharged on day 29 after surgery.To summarize the diagnosis and treatment experience for this case,ALPPS is a feasible option for the treatment of spontaneous rupture of HCC.However,ALPPS could induce intraoperative or postoperative rebleeding of ruptured tumours.This regimen should be chosen very carefully.If postoperative complications cannot be controlled,this method should be avoided.关键词
