Efficacy of novel plasma needle electrode for en bloc resection of bladder tumor in treating elderly NMIBC and factors influencing postoperative recurrence

Abstract
Objective To investigate the efficacy of en bloc resection of bladder tumor(ERBT)using a novel plasma needle electrode in elderly patients with non-muscle invasive bladder cancer(NMIBC)and the factors influencing postoperative recurrence.Methods Clinical data of 134 elderly NMIBC patients who underwent ERBT with a novel plasma needle electrode at Beijing Friendship Hospi-tal,Capital Medical University from March 2021 to November 2023 were retrospectively analyzed.Univariate analysis was used to explore the association of gender,age,body mass index(BMI),comorbidities,smoking history,alcohol history,pathological diag-nosis,T stage,tumor grade,tumor location,and tumor number with recurrence.Cox regression analysis was further employed to identify independent factors influencing postoperative recurrence and to analyze the impact of each variable on recurrence-free cumula-tive survival rate.Results All patients successfully underwent surgery with satisfactory tumor resection and no serious complica-tions.Within one year postoperatively,98 patients remained recurrence-free,while 36 experienced recurrence,yielding a recurrence rate of 26.87%.Univariate analysis showed statistically significant differences between recurrence and non-recurrence groups in age,combined hypertension,combined diabetes mellitus,T stage,tumor grade,and tumor number(P<0.05).Multivariate analysis i-dentified age,combined hypertension,combined diabetes mellitus,T stage,tumor grade,and tumor number as independent factors influencing postoperative recurrence(P<0.05).Significant differences were observed in recurrence-free cumulative survival rates a-mong patients with different ages,combined hypertension status,combined diabetes status,T stages,tumor grades,and tumor numbers(P<0.05).Conclusion ERBT with a novel plasma needle electrode demonstrates favorable efficacy in elderly NMIBC pa-tients;however,the tumor recurrence rate within one year postoperatively remains relatively high.Age,combined hypertension,combined diabetes mellitus,T stage,tumor grade,and tumor number are independent factors influencing postoperative recurrence.Enhanced preoperative assessment,optimized comorbidity management,and close postoperative follow-up should be emphasized for early intervention of recurrence.关键词
