Typical Models of Integrated Care for the Elderly in China:A Comparative Analysis and Optimization Path

Abstract
Integrated care for the elderly is a key exploration in actively responding to the national strategy on population aging and constructing a Chinese-style eldercare service system.Using a comparative case study method,this paper systematically compares three typical models from the perspectives of integration form,integration degree and integration dimension:the government-led medical-nursing integration model,the multi-stakeholder collaborative community integration model,and the market-driven institutional integration model.It analyzes the operational logic,common features,and heterogeneous characteristics of these three models,as well as the challenges in development and conditions for their promotion.The study finds that the three models follow distinct operational logics:"hospital-driven,management-fused,and closed-loop service""stakeholder-collaborative,resource-integrated,and linked-service"and"hierarchically-structured,IDT-penetrated,and case-managed",respectively.All models embody the core principles of integrated care,including being person-centered,horizontal integration,interdisciplinary teamwork,and personalized care,yet they exhibit significant differences in the degree and dimensions of integration.In their development,the promotion of these models face challenges such as inter-departmental coordination barriers,inadequate sustainable payment mechanisms,and insufficient integration of information technology.Based on these findings,it is proposed that systematic policies should be implemented across five areas:improving top-level design,innovating management systems,strengthening financing and payment mechanisms,enhancing professional team building,and deepening the empowerment of smart technology,so as to provide strong support for the high-quality development of China's eldercare.关键词
