针灸治疗自身免疫性GFAP脑炎尿失禁1例
Acupuncture Treatment of Urinary Incontinence with Autoimmune GFAP Encephalitis:A Case Report

Abstract
Encephalitis,a severe inflammatory condition of the brain,autoimmune encephalitis is one type of encephalitis,encompasses a variety of etiologies,the pathology mainly involves diffuse damage throughout the entire brain.The clinical symptoms include headache,fever,behavioral alterations,psychiatric symptoms,mood disorders,memory impairment,seizures,motor dysfunction,autonomic nerve instability,consciousness disorders,sleep disorders,and hyponatremia.This case report detailed the clinical course of a young male diagnosed with autoimmune encephalitis,marked by an elevated 24-hour intrathecal synthesis rate of immunoglobulin G(IgG)in the cerebrospinal fluid,glial fibrillary acidic protein(GFAP)antibody was positive,and the enhanced magnetic resonance imaging(MRI)scan of the skull revealed irregularly shaped thin linear images that were perpendicular to the ventricles.During the onset of the illness,the patient mainly exhibited fever,needle-like headache,generalized myalgia,diarrhea,and behavioral abnormalities.In the hospital setting,the patient's condition deteriorated,manifesting as behavioral disturbances,disorganized speech,inability to accurately answer questions,seizure-like episodes,respiratory alkalosis,pulmonary infection,bloodstream infection,tracheotomy incision infection,gastrointestinal infection,liver dysfunction,and electrolyte imbalances(hyponatremia,hypokalemia).The patient's consciousness was significantly impaired with a low likelihood of rapid recovery.A comprehensive treatment regimen was initiated,including antimicrobial therapy,intracranial pressure management,antiviral agents,anti-liver injury,corticosteroids anti-inflammatory,and anti-fungal treatment.This approach led to substantial improvements in the patient's consciousness,psychiatric symptoms,infection,and liver function injury.However,nearly two years after leaving the hospital,the patient still had mental symptoms,their personality had changed,and urinary incontinence had not fully recovered.Following a two-month course of acupuncture therapy,the patient's psychological status showed marked improvement,and urinary incontinence was significantly alleviated.This case highlights the potential therapeutic benefits of acupuncture in autoimmune encephalitis,suggesting its potential role in the management of central nervous system diseases.However,the efficacy and generalizability of acupuncture require further substantiation through rigorous clinical research and evidence-based data.关键词
